BMI Explained: What It Measures and What It Doesn't

What BMI actually calculates
Body Mass Index is a single formula: weight in kilograms divided by height in metres squared. That's it — no measurement of body fat, muscle, or composition goes into it directly. It became widely used because it's cheap and fast to calculate from just two numbers, and at a population level it correlates reasonably well with health risk. The standard adult categories (for ages 20 and up) are underweight below 18.5, healthy weight from 18.5 to just under 25, overweight from 25 to just under 30, and obesity at 30 and above, per the CDC.

Those category boundaries were derived from population-level data — they describe where health risk tends to rise across large groups of people, not a verdict on any one individual's health. The CDC's own framing treats BMI as a screening tool, meant to be considered alongside other factors like a physical exam, lab results, health behaviors, and family history — not as a standalone diagnosis of anything.
What the CDC says BMI can't tell you
The CDC states this directly in its own FAQ: "BMI is not a direct measure of body fat. BMI cannot distinguish fat mass from lean body mass (muscle and bone). BMI also cannot indicate where fat is located in the body." That's why someone with a lot of muscle mass can register as "overweight" on BMI without having excess body fat at all — the formula has no way to tell the difference between muscle and fat, or between fat carried around the waist versus elsewhere, even though those distinctions matter for health risk.

This is one of the clearest examples of why a single set of BMI thresholds doesn't apply uniformly to everyone.

In June 2023, the American Medical Association adopted a new policy on BMI, stating that it "does not account for differences across race/ethnic groups, sexes, genders, and age-span" and that it "loses predictability when applied on the individual level." The AMA recommends using BMI alongside other measures — visceral fat, body composition, waist circumference, or relative fat mass — rather than as a sole criterion, and specifically states it shouldn't be the only basis for decisions like insurance reimbursement.
Better together: pairing BMI with other measures
Waist circumference is one of the more commonly recommended companion measures, since it reflects abdominal fat in a way BMI can't. The National Institutes of Health flags increased health risk above roughly 35 inches for women and 40 inches for men. None of these measures is a diagnosis on its own — they're inputs a healthcare provider can use together, alongside your broader health picture, to get a fuller read than any single number provides.
